IF YOU HAVE ANY DEADLINES COMING UP THEN PLEASE REQUEST AS SOON AS POSSIBLE.
PLEASE DO NOT REQUEST A TRANSCRIPT UNTIL YOU HAVE APPLIED TO YOUR COLLEGE/UNIVERSITY
CURRENT STUDENTS: You may pick up a Transcript Request form from Mrs. Garner in the Counseling
Department. If you have already requested a Transcript this year then please let her know, you will
continue to request transcripts on the same form you originally filled out. If you are sending a Transcript
to a college/university in Texas then those will be sent through our TREX system electronically. If you are
sending to a college/university out of state then I will call you down to pick it up so that you may send
it to them directly. The first 5 are free, beyond that each transcript is $1.00 and must be paid before
request will be processed.
GRADUATES & FORMER STUDENTS: You can obtain a transcript one of two ways. You may either come
to the school in person to fill out the Transcript Request form or you can click here
to print, fill out and fax to 281-577-9174 with a copy of your ID. Please note that there is a $1.00 charge for each Transcript that is requested and must be paid at the time of the request.
COLLEGES/UNIVERSITIES/EMPLOYERS: You may obtain a copy of a current or former students’
transcript by mailing, emailing or faxing a signed release form from a student giving Porter High School
permission to release their transcript.
